Here's what I use:

 © 1999 Digital Equipment Corporation.  All Rights Reserved.

Use a single date if the item is "new".

If the item has been copyrighted for two years, use both dates, 
separated by a comma:

 © 1998, 1999 Digital Equipment Corporation.  All Rights Reserved.

If the copyright spans several years, use a dash to indicated the
range:

 © 1994-1999 Digital Equipment Corporation.  All Rights Reserved.
